Order form for pet portraits

Fill in the form and send it to me by clicking on the postbox below.

You can either send photos of your pet in the post, or as an email attachment seperate from the form. The format must be jpg or gif and at the highest possible resolution. This is so that I can print out the photo and get a good amount of detail in the features.

Prints of any pet portrait can be purchased for £10 each excluding postage
Or why not have a tshirt with your own pet portrait or one from the gallery

A deposit of 20% of the total cost is required before I start the artwork, and the balance is payable on completion. Payment can be made either through Paypal using any major credit card, cheque, or registered cash.

I will return any photos you send me with the completed artwork.
